ACT Rehabilitation Specialist

Essential Duties and Responsibilities

As a team member on Lifewell’s ACT Team, you will be responsible for providing customized community-based services for people living with a serious mental illness who need more intensive wrap-around services to keep them living their lives in the community and out of the hospitals, jails, crisis system or emergency rooms.

Specifically, as an ACT Rehabilitation Specialist you will provide direct treatment, rehabilitative and supportive services in a community setting to members. You will work as part of a clinical team to assess member’s needs and wants, participate in the development of individual service plans and acts as a member advocate and liaisons with families and community agencies. In addition you will routinely assess member’s need for ACT services, utilize outside services (probation/parole, court order treatment and change services if needed. You will be responsible for providing on-call crisis intervention services, as well as weekend and holiday coverage and support the acquisition of Independent Living Skills through direct skills training or in collaboration with other team members.

Skills / Requirements

  • A High School Diploma/GED and three years of relevant experience in a behavioral health setting; OR
  • An Associate’s degree and two years of relevant experience in a behavioral health setting; OR
  • A bachelor’s degree in the behavioral health field; OR 
  • A non-behavioral health field bachelor’s degree and one year of relevant experience in a behavioral health setting
  • A combination of Certified Psychiatric Rehabilitation Practitioner (CPRP) certification and one year of behavioral health work experience; in a rehabilitation setting preferred
  • Must be at least 21 years of age.
  • Background check.
  • Minimum typing speed of 25 wpm.
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
