Lifewell seeks a compassionate and progressive family practice or internist primary care NP who wants to make a difference and work in the innovative field of primary care integration! We are offering a $3,000 Sign-on and Retention Bonus.

 

As Lifewell’s Family Nurse Practitioner, you will work at our outpatient clinics in the Phoenix area and treat patients with a serious mental illness (SMI). You will be responsible for the full scope of primary care services to adults diagnosed with a serious mental illness. You will oversee daily clinical practices of integrated primary care services and work collaboratively with the Clinical Team to develop and implement healthcare outreach and healthcare programs to reduce gaps in care and improve primary care and wellness service offerings. You will deliver Medication Assisted Treatment (MAT) services, as appropriate and complete timely and accurate documentation of observations, interventions, plan of care, referrals and follow-up.
