Job Description

POSITION SUMMARY: Under supervision of the Senior Director, plans, organizes, and directs the activities of the Lifewell High Risk and other assignments in providing clinical therapeutic services to an adult population composed of substance-abusing, SMI, dual-diagnosed and/or referred persons. Provides direction and support to team members in the management of member risk episodes and resulting coordination. Responsible for the overall management of high risk caseloads, special assistance, and other populations as needed; performs related duties as required.


  • Develops, implements, and evaluates high risk monitoring requirements and training.
  • Works collaboratively with leadership and clinical teams in monitoring high risk caseloads through review and knowledge of the Lifewell reports server, HIE data, Care Quotient data, health plan reports, and other high risk information.
  • Serves as an expert and supports clinical teams in compliance with all Lifewell high risk requirements.
  • In coordination with leadership and quality department, implements new processes, concepts or methods that will improve the quality of member care provided to members identified as high risk to improve health outcomes and member experiences.
  • Works with health plans, HIE, and other data sources to obtain and monitor high risk data.
  • Maintains a system of communication and reporting between all staff members, sites, and departments involved with the ongoing maintenance of high risk coordination.
  • Establishes, facilitates and evaluates objectives and goals for high risk compliance.
  • Conducts meetings to discuss plans, outline new goals and objectives, review areas of concern, and disseminate information.
  • Recommends, defines and develops annual goals, objectives and short- and long-term plans related to high risk coordination.
  • Coordinates, communicates, and collaborates with other components of the organization and outside agencies; seeks mutual agreement on problems involving coordination; consults with all segments of management responsible for policy or action.
  • Fosters strong community relations; maintains productive working relationships with neighbors, landlords, area professionals and organizations to coordinate member services; participates in community and professional activities to promote the work of the organization.


  • A bachelor’s degree in the behavioral health field and a combination of education and full-time relevant experience in a behavioral health setting totaling four years
  • Preferred: A master’s degree in the behavioral health field
  • Preferred: Licensure issued by the Arizona Board of Behavioral Health Examiners to engage in the practice of behavioral health in Arizona at the Associate, LISAC or Independent level.
  • Some assignments require an Arizona DPS Class One Fingerprint Clearance Card
  • Some assignments require a current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
  • Must be 21 years of age
  • Background Check
  • Minimum 1 year recovery from drugs and/or alcohol
  • Minimum typing speed of 25 wpm
