ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Conducts screenings and assessments with behavioral health recipients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate service plan goals.
  • Develops and provides initial individual service plans; schedules and/or arranges appropriate ongoing treatment according to the behavioral health recipient’s needs and wishes, provides internal and external educational resources.
  • Prepares intake documentation, progress notes, event notifications and other documentation regarding behavioral health recipients in accordance with regulatory requirements and company policies and procedures.
  • Ensures required documentation is completed in accordance with established timelines; including, but not limited to, screenings, assessments and service plans.
  • Facilitates coordination of care, to include: notification to program staff of new behavioral health recipient intake, summary of intake information and recommended services/programs.
  • Participates in clinical staff meetings, trainings and other activities as required.

 

KNOWLEDGE, SKILLS AND ABILITIES REQUIREMENTS

  • Knowledge of the current DSM diagnoses, psychopharmacology, substance abuse, addictions, recovery, relapse prevention, and 12-step treatment philosophy.
  • Knowledge of the principles of cultural competency and adherence to its philosophy.
  • Knowledge of Lifewell’s Code of Ethics statement and adherence to its concepts.
  • Knowledge of Lifewell’s behavioral health and related services delivery systems policies and procedures.
  • Knowledge of Arizona behavioral health services delivery system policies and procedures.
  • Knowledge of state behavioral health individual and agency licensure regulations and requirements.
  • Knowledge of state and local behavioral health resources.
  • Knowledge, skill and ability to complete comprehensive assessments accurately.
  • Ability to work effectively with others and to communicate effectively, orally and in writing.
  • Ability to maintain confidentiality of information.
  • Ability to utilize good judgment, decision-making and work independently.

Ability to work under pressure and address events/crises which may develop unexpectedly.

Skills / Requirements

EDUCATION/LICENSE/CERTIFICATION/EXPERIENCE REQUIREMENTS

  • Licensure issued by the Arizona Board of Behavioral Health Examiners to engage in the practice of behavioral health in Arizona.
  • Intake Specialist I: Licensed at the Associate level
  • Intake Specialist II: Licensed at the LISAC level.
  • Intake Specialist III: Licensed at the independent level.
  • A master's degree in counseling, social work, psychology or a closely related behavioral health field and two years of professional counseling experience in a substance abuse or mental health treatment program; OR any equivalent combination of education and experience.
  • An Arizona DPS Class One Fingerprint Clearance Card or the ability to obtain one.
  • A current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
    • Must be 21 years of age
    • Background Check
    • Minimum 1 year recovery from drugs and/or alcohol
    • Minimum typing speed of 25 wpm
    • Possession of, or the ability to obtain, the following:
    • CPR and First Aid certifications
    • Evidence of freedom from infectious pulmonary tuberculosis

    • Standard office environment and mobility to travel between various organizational worksites and/or external meetings.

    • Ability to create and maintain required documents.
